I trace how colonial priorities never fully disappeared; they simply shifted into modern forms of Western influence over funding, research, and public health decisions. Through examples like donor-driven HIV/AIDS programs and foreign-led clinical research, the episode shows how external institutions continue to decide which lives and diseases receive attention, often sidelining local needs. I also outline what it would take to return real control to Tanzanians themselves: stronger local governance, investment in domestic research capacity, ethical ownership of knowledge, and community-centered healthcare. What this really means is that the struggle for health in Tanzania is also a struggle for agency, and the path forward depends on giving that agency back.
