LVOT obstruction is defined as a peak gradient of at least 30 mm Hg at rest or with provocation. The first documented stopped heart surgery was performed on February 17, 1956. Dr. Lars Svensson, Chairman of the Heart, Vascular and Thoracic Institute reviews the operative notes from this innovative surgery performed by Dr. Donald Effler and Dr. Lawrence Groves, using a heart lung machine developed by Dr. Willem Kolff at Cleveland Clinic. Dr. Svensson uses illustrations to take us step by step through the procedure. The majority of heart procedures done today use a similar process to stop the heart as the one that Drs. Effler and Groves used 65 years ago during this ground-breaking surgery.

It is important to ask the right questions and look for risk factors to determine if there are underlying sleep disorders in the patient with cardiovascular disease. The STOP-BANG questionnaire can be used to test for high probability for sleep apnea but there are limitations. Testing for sleep apnea can be done at home – or in lab; and there are many ways it can be treated. Treating sleep apnea can improve cardiovascular disease, especially atrial fibrillation and high blood pressure.

Those that are more fit are more likely to maintain long term sinus rhythm, come off antiarrhythmic medications and have improved mortality. This study reinforces the impact that cardiovascular risk factors such as weight, blood pressure, diabetes, sleep apnea - and now cardiovascular fitness has on atrial fibrillation management.
