Have a VA doctor evaluate you, have them recommend the program and have a 50% or more SCD% rating. That is the basics of the program although it can be hard to navigate and that’s where I come in. There she focused on educating community members, especially caregivers, through marketing, planning, and communication.
In March of 2023, her dad was diagnosed with Early Onset Alzheimer’s – with that and feeling the need to explore her calling as a death doula – she took a leap of faith and started her own marketing business with a goal to be able to allow flexibility to help support her dad (and mom) and start diving into deathcare.

She has worked in various settings from long term/skilled care, home health and hospice, Alzheimer's support group facilitator, and most recently working to coordinate care for those receiving services through Utah's Medicaid aging waiver. Annie also started Senor Care Coordination of Northern Utah to assist families with difficult decisions regarding care for their family members requiring some form of long-term care.

Annie retired at the end of September 2023 and moved her elderly mother into her home to become her full-time caregiver. Annie and her mother enjoyed 4 months together, prior to her mother's passing in January.

Robert Drake is an interfaith chaplain, life coach, grief counselor and end of life doula who now operates a Drake Living & Dying Design on his six acre farm on Washington's Olympic Peninsula. Originally trained as a philosopher, Robert has a Master of Science degree in philosophy and conflict resolution and Master's of Divinity with experience as a hospital, hospice and palliative care spiritual care provider and grief counselor.

Home Health is Not Home Care. Home Health provides clinical or skilled needs.
Home Health could be a Nurse or Therapy, such as Physical, Occupational etc.
Patient is recovering in the home from a broken hip or a stroke.
Should you go to Skilled Nursing or Home Health? That is a decision for each Patient and their family.
Home Health is not in the Home 24 hours a day. You will also need Custodial Care, which is Home Care or a family member.
Home Health is set up via your Doctor.
How is Home Health Paid? it is paid by Medicare and possibly some private insurance.
You can also receive Home Health in an Assisted Living.

Hospice---
Quality and Comfort Care at end of Life.
When do you start the conversation about Hospice? When there is 6 months or less, life expectancy.
Hospice is not in the Home 24 hours a day. Someone else must be in the Home with the Patient. Home Care or the family.
A Doctor must refer the Patient to Hospice.
What happens after 6 months, can Hospice Care last years. Yes it can.
Can you Graduate from Hospice Care? Yes you can!
Hospice Care can happen in the Home, the Hospital, Assisted Living, Skilled Nursing, etc.
Medicare Pays for Hospice!! Medicaid will also pay for Hospice.

I am currently the hospice clinical manager and have a masters degree in Nursing Leadership and Management. In my opinion, hospice is the best nursing field, but I am biased! Hospice nurses are the navy seals of the nursing profession. They exhibit some of the highest levels of critical thinking required to guide patients to end of life. I am proud to be a nurse and even more proud to be considered a hospice nurse.

Dani Barnes, has worked with St. Luke’s Hospice for a little over 9 years now. For years, I oversaw all of our clinician’s schedules and referral intake. I was able to form a close relationship with our social workers and case managers. I have gained an immense amount of empathy and respect for what they do every day. If the health concerns are verified during the visit
When you get back home--talk with siblings and reach out to Mom, Dad etc.!

Folks, I know it is a bad four letter word, "Plan"!!!

She initially may need 24-hour care, for a week, for many weeks or more!
Who will care for her? How much will it cost?
Care in the Home. Care at an Assited Living but you could be paying two bills!!
$7,000 to $29,000 a month!! It could be that much! Do they have that much money?
Where does the money come from? Their piggy bank, the family piggy bank, a Long-Term Care Insurance Policy, a Reverse Mortgage, the Veterans Administration.
This scenario is appliable to a broken hip, a stroke or an illness!!
